Clinical tool gathers momentum across sector

Since its launch in November 2013, Western Sydney HealthPathways continues to gain momentum across the region as health professionals in both primary and secondary care utilise, explore and support the development of the clinical portal.

With 339 pathways live, and two thirds of GPs surveyed in western Sydney now using HealthPathways, the portal provides a wide variety of locally relevant information and resources aimed at enhancing patient management and referrals.

To date over 280 western Sydney health professionals have participated in the development of pathways across a wide variety of health conditions and specialist areas. Each pathway has been created using the expertise and experience of local GPs, specialists, nurses and allied health providers, to improve access to care.

With a newly released pathway on Breastfeeding, the top five most popular pathways viewed across western Sydney are:
·National Disability Insurance Scheme (NDIS)
·Screening and Diagnosis of Diabetes
·Rapid Access and Stabilisation (RAS) Service
·Obesity Management

Dementia Care Forum and Quality Award

As part of Dementia Awareness Month in September, the Western Sydney Dementia Strategy Implementation Group (WSDSIG) is hosting a informative forum about Dementia. The day's event is for community workers, people living with Dementia and their families, carers, health professionals and aged care facility staff. Click here for more information. EOI: Digital Health Advisory Group

During a time when the focus on digital health is steadily increasing, Western Sydney Primary Health Network (WSPHN) is seeking expressions of interest from key groups to form a Digital Health Advisory Group.
