Game Day on health care reform for contractor and employee GPs

How match-ready are you for the impending health care home reforms in primary care.

Next Wednesday evening, WentWest – the Western Sydney Primary Health Network (WSPHN), will be hosting a unique forum centring on the implications of health care reform for contractor and employee GPs within western Sydney. Featuring diverse speakers from Tonic Health, Avant, the Australian Digital Health Agency and local practices, the WSPHN and its partners will explore the evolving general practice landscape in Australia, working towards a vision for how it may look into the future.

With host and renowned health personality Dr Norman Swan facilitating discussions, the evening will also discuss the impact of current health care reform and the influence of the Health Care Home model on GPs working within local practices.

For avid football fans, the forum will conclude with hearty refreshments and networking while State of Origin plays on the big screen.

Dedicated Coaching: Stanford Chronic Disease Management

Studies show that 99% of patient care is done by the patient and having a structured self-management program is able to significantly impact clinical metrics like HbA1c and blood pressure. To support this, the Western Sydney Local Health District and the Western Sydney Primary Health Network are offering practices the opportunity to train peer leaders in the Stanford program. The program developed in the USA, is a peer coaching model that has delivered marked results to people with a lived chronic disease.
