New App set to support patient self-management

With time constraints ever-present, accessing general practice tools which can seamlessly fit into practice workflow while also improving self-management is a potential key to improving and sustaining care and support for patients.

In response to this Western Sydney Primary Health Network (WSPHN), in partnership with Western Sydney Local Health District, is pleased to announce the launch of GoShare Healthcare – a proven patient education and content distribution application aiding GPs and practice nurses to send tailored health information to patients via email or sms in seconds as part of a routine consultation.

Providing access to credible, evidence-based resources that can be easily customised to meet individual patient needs, GoShare Healthcare includes: access to 800+ patient & carer stories about managing certain illnesses, animated information videos, 1000+ information sheets, and tools & resources.

GoShare Healthcare is one component in a suite of integrated, digital health services which the PHN offers, with several more to be announced shortly. Together the suite of services work to enhance online support to local practices in providing patients holistic and coordinated care.

GoShare HealthCare is available free of charge to a limited number of practices. To find out more about the services click here or contact the HelpDesk on (02) 8811 7117.
